This question type allows you to capture a respondent’s name.
Options
Here’s what the name question type looks like in the survey editor:
Include a note – Checking this will allow you to enter a note that will appear under your question.
Question is mandatory – Checking this will make the question mandatory, meaning that it must be answered before the participant can complete the survey.
Display Fields – Changing the name format on the dropdown determines whether title and suffix are captured. You can choose from:
- Full name
- Title, first and last
- First and last
Note that you can add media to the the name question type.
